Please read before submitting photos:

1.)  Pictures that are blurry will not qualify for the gift card. Please send quality photos only.

2.)  Pictures may be used online, if you have any objections to family pictures being advertised on our site, please do not submit them on this form.

3.)  If the decal layout is edited or changed during application and does not reflect the same look and style of the online design, it will void the gift card terms and will not qualify.

4.) Pictures must be of our catalog designs ONLY. This means that custom designs created through the custom design teams will not qualify for this gift card promotion.

Terms - Please Read Carefully

* One gift card per design picture. Sending in multiple pictures of the same design will not warrant multiple gift cards.

* Please give us up to 5 - 10 business days to process the photo submission before the gift card is emailed.

* Pictures submitted on the same form will be combined into 1 gift card. If multiple forms are submitted, then multiple gift cards will be issued.

* Because only 1 code can be used at checkout, this gift card code cannot be combined with any other sale, discount, or promotions including Groupons. There will be no exceptions to this policy.

Photo Authorization Release:

I, the submitter of this form, do hereby consent and agree that GLS Signs LLC has the right to use the attached photograph(s) and to use these in any and all media, advertising, or website presence, now or hereafter known, and exclusively for the purpose of product advertising. I further consent that my name and identity may be revealed therein or by descriptive text or commentary.

I do hereby release to GLS Signs LLC, all rights to exhibit this work in print and electronic form publicly or privately and to market and sell copies. I waive any rights, claims, or interest I may have to control the use of my identity or likeness in whatever media used.

I understand that there will be a one-time gift card of the advertised amount that will be issued per decal design.

I also understand that GLS Signs LLC is not responsible for any expense or liability incurred as a result of my participation in this photograph.

I represent that I am at least 18 years of age, have read and understand the foregoing statement, and am competent to execute this agreement. I further claim that I am the rightful owner of this original photograph and am giving GLS Signs LLC Photo Submission.
